Leave of Absence Request Form (if your child needs to be taken out of school for any reason other than through illness eg sporting activity, music exam, holiday, etc then a Leave of Absence Request Form must be completed. This is only available on request from the School Office).
NB: Holidays taken in Term time will not be authorised. Absences in term time will only be authorised in exceptional circumstances. If a holiday is taken in term time then the absence will be classed as unauthorised and a Fixed Penalty Notice should be issued.
